ADHD Preparation & Support Tool
Reflect on ADHD-type traits, gather real-life examples and create a clear summary to take to your GP, school, employer or assessment service.
This tool does not diagnose ADHD. It helps you organise information about attention, impulsivity, hyperactivity, emotional regulation and daily functioning. A diagnosis of ADHD can only be made by an appropriately qualified healthcare professional following a full clinical assessment.
Questionnaires and summaries from this tool are not a diagnosis. They are designed to help you prepare for conversations with your GP, school, employer, university or clinician.
